Pride of the Wolfpack Award Notification

This form should only be submitted by the Pride of the Wolfpack Award coordinators.

Complete the below information once an employee has been selected for the Pride of the Wolfpack Award. Upon verification of eligibility, Learning and Organizational Development will process the NC State Bookstores to process award certificates and mugs for all qualified recipients.

Awards are ordered at quarter-end, and honorees are listed on the Pride of the Wolfpack homepage quarterly.
